We want to welcome you as a new patient to PCSG!  In this section, we’ll provide some information and details that will help you prepare for your first appointment.

Complete your New Patient Packet

As a new patient, you are able to download our New Patient Packet and complete the patient forms in the convenience of your home.  Then, just bring them with you for your first appointment. Many of our patients tell us they like being able to complete the forms at home!  To access the forms, please click here.

Preparing for Your First Appointment

As you arrive for your first appointment, we would like to ask that you arrive at least 15-20 minutes before your appointment time.  This gives us time to input your insurance information, new patient forms, patient information and more.  Additionally, please make sure to bring a list of any medications that you are taking currently.   The first appointment may take a little extra time, but we promise to make this first appointment seamless as possible.  Thank you for your patience!

Exploring the Benefits of the Patient Portal

You will now have access to PCSG’s Patient Portal, which allows you to have your health information right at your fingertips.  You can send secure messages to your Provider, request prescription refills, find your lab and test results, and much more.  Plus, the Patient Portal is available 24 hours a day, 7 days a week and 365 days a year!  Anytime that you need to access your information, it’s available to you.  We encourage our patients to sign-up for the Patient Portal and utilize it throughout the year as needed. 

After Hours Care

Did you know that PCSG offers after-hours coverage to our patients?  That’s right! A physician is always available by phone after hours for both adults and children. Our Providers may be reached by calling the number of the office at which the patient is seen. All phones are forwarded to an answering service at the close of each business day, on weekends, and holidays.  Our goal is to always provide access, and even expand access to your provider, so you can always reach PCSG when you need us most! We’re here to care for you – 24 hours a day, 7 days a week, 365 days a year.
