INSURANCE INFORMATION AND SLIDING FEE PROGRAM

 

PCSG’s mission is to improve the health of the population in Southwest Georgia by providing patient-centered, preventative, primary health, behavioral health, enabling and health education services, with an emphasis on the medically uninsured and underserved, regardless of the patient’s or the family’s ability to pay.

 

Major Medical Insurance Plans

PCSG accepts most major insurance plans, including Medicare and Medicaid.

 

Please note that all PCSC patients are responsible for payment of any co-pays and/or co-insurance at the time of service.

 

Sliding Fee Program Available for Qualifying Patients

The healthcare services you deserve and receive are not based on your ability to pay. PCSG offers a discount program that allows
individuals with no insurance or a low income to receive health care at a discounted rate based on income status. To determine eligibility, you are required to provide proof of income for all household members. The amount of the co-pay will be based on the information provided.

 

Documents that are acceptable for proof of household income include:

a)   W2 or Income Tax Return from the previous year

b)   Two consecutive, current check stubs or paychecks (copies are acceptable)

c)   Letter from current employer

 

Patients using this program will be expected to pay their Sliding Fee Program co-pay at the time of service. Please feel free to contact one of our locations to learn more about our Sliding Fee Program.

 

Self-Pay Patients

Individuals who do not have insurance, and do not qualify for the Sliding Fee Scale program, can still receive care at PCSG,
however you are responsible for the full charges of your visit.  Please feel free to contact one of our locations to learn more about the Self-Pay option.
